Posterior Tibialis Tendon Dysfunction (PTTD)

A Common but Overlooked Cause of Adult Flatfoot & Inner Ankle Pain

What’s Going On?

PTTD occurs when the posterior tibialis tendon—responsible for supporting your foot’s arch—becomes overstretched, inflamed, or torn.
This leads to gradual flattening of the arch and rolling in of the ankle (overpronation), often causing pain and swelling along the inside of the ankle or foot.

It typically affects adults over 40 and is more common in women and people who spend long periods standing or walking.

Common causes include:

  • Overuse or repetitive strain
  • Flat feet or poor foot posture
  • Weight gain or obesity
  • Tight calf muscles or Achilles tendon
  • High-impact or stop-start sports
  • Previous ankle or foot injury

What We Did Today

  • Assessed your foot and ankle structure, strength, and flexibility
  • Identified signs of tendon strain or arch collapse
  • Reviewed footwear and activity levels
  • Discussed contributing factors such as posture or calf tightness
  • Developed a treatment plan to reduce load and support the tendon

What Happens Next

Your management plan may include:

  • Custom orthotics – to restore arch support and reduce strain
  • Supportive footwear – firm, stable shoes with arch control
  • Activity modification – reduce high-impact activity while symptoms settle
  • Targeted strengthening – exercises for the foot and calf to restore function
  • Taping or bracing – short-term support to offload the tendon
  • Shockwave or laser therapy – to stimulate healing and reduce pain
  • Manual therapy or dry needling – to relieve tight, compensating muscles
  • Referral to specialist – if structural damage or arthritis is advanced

Follow-up:
PTTD is a progressive condition. Early treatment can prevent long-term arch collapse and maintain mobility.
Book a review if pain persists or if your foot shape or balance continues to change.
